Our Practice:


Northwest Medical Care is a Primary Care/Internal Medicine practice started by Dr. Mullengada in 2005 with a desire to provide comprehensive but personal medical care.  The desire to have an old school medical practice with enriching doctor-patient relationships & the need for work life balance led to the start of Northwest Medical Care.

We are located in Succasunna, Roxbury Towhship in New Jersey.  Northwest Medical Care started with a single provider and has added on two advance-practice nurses (APNs) to provide better care & availability.  Our Nurse Practitioners, Denise B. Cusick  and Sarah L Hagemeister specializes in Family Practice.  We provide team based care.  You will see both the Physician and Nurse practitioners based on your needs.  The Nurse Practitioners do collaborate closely with Dr. Mullengada.

Our team is made complete with our excellent Certified Medical Assistants (CMAs), Care-Coordinator & Billing Specialist.  We are dedicated to making every visit to our practice a comfortable & healthy experience.

Northwest Medical Care is Participating in a New Program to Enhance Your Care: Primary Care First

For regular in-office visits, we will be maintaining strict precautions to limit exposure to infections.


  • Strict social & physical distancing measures will be maintained.

  • We request you to wait in your cars instead of our waiting room. Please call us on your arrival, we will check you in and have you come straight to an exam room when it becomes available.

  • Exam rooms will be cleaned & disinfected between patients consistently.

  • Masks/face covering will be mandatory for all patients on entering our office.

  • All employees will be screened daily for fever & COVID related symptoms.

  • Please restrict companions during your visits unless medically necessary.

We will continue to provide and maximize Televisit/Telehealth services for as long as Insurance covers it.

It is also important that if you are having COVID symptoms, please call ahead of your visit to communicate symptoms before arriving, enabling the provider to determine the best setting for you to receive care.
